Step 1: Schedule Your Appointment

Step 2: Complete Pre-Examination Paperwork

Once scheduled, we will send you the forms electronically via text message or email for you to fill out before you come in for your appointment to allow for an easy and convenient process.

Before arriving for your appointment, make sure you bring with you a valid driver's license, any relevant medical records/history, a list of your current medications, and any corrective lenses, contacts, or hearing aids.

Step 3: Physical Examination & Testing

Dr. Altavilla will review and clarify your answers on the pre-examination paperwork you completed, and then he will perform all federally required components of the DOT exam — including vitals, vision, hearing, urinalysis (NOT A DRUG TEST), and a comprehensive physical evaluation.

Step 4: Medical Review and Clearance Decision

Your results are reviewed immediately, and Dr. Altavilla determines your medical certification status based on FMCSA guidelines.

Step 5: Receive Your DOT Medical Card

If approved, you’ll receive your DOT Medical Certificate the same day, along with guidance on renewal timelines and any follow-up requirements.
